Qualifications:

• As required, offer guidance and counseling to the client’s family and/or other caregivers to encourage the client’s progress toward a discharge plan and mutually agreed-upon goals.
• Collects data on a patient’s physical and mental health to inform treatment planning and progress monitoring.
• Stop the development of impairments, functional restrictions, and disabilities brought on by illnesses, disorders, ailments, or traumas.
• To help patients cut back on their drug use and prevent surgery, patients should be given information and training.
• Restore, maintain, and encourage optimal physical function, as well as optimal wellness, fitness, and quality of life, in terms of movement and health.
• Make a plan for the patient’s care, make sure they stick to it, and alter it as needed based on how well they’re doing.
• By working together with the patient’s physician and other healthcare providers, the therapist is able to learn more about the individual, modify the treatment plan as needed, and better incorporate physical therapy into the patient’s overall care.
• Recognizes the value of different reimbursement models and effectively allocates therapy resources to residents in accordance with their discharge destination objectives.
• Instructs, stimulates, and aids the patient in a variety of physical tasks, such as non-manual exercises, ambulatory functional activities, daily-living activities, and the use of assistive and supportive equipment such as crutches, canes, and prostheses.
• It is necessary to finish initial and ongoing physical therapy evaluations in accordance with APTA standards and state regulations.

Skills:

• Qualities such as empathy, compassion, and the ability to inspire patients to push themselves safely and constructively are essential.
• The ideal applicant will have a strong sense of leadership and a never-ending hunger for knowledge and skill development.
• Experience working in a nursing home, hospital, or inpatient/outpatient setting for at least a year is preferred.
• We are looking for candidates with experience in a rehabilitation, Orthopedics, or medical/surgical setting who can demonstrate their knowledge, skills, and competencies for the position.
• One must have completed a program leading to a degree in physical therapy that has been recognized by the American Physical Therapy Association.
