Job Description

The Medical Record Clerk is accountable and liable for maintaining health records. The individual is responsible for organizing and maintaining a system of health files and logs pertaining to patient diagnosis, treatment, admissions, discharges, and daily patient census changes. The employee is required to act independently and with good judgment and confidentiality.

  • Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
  • Closed records will be purged by the medical record clerk in preparation for boxing and off-site storage.
  • Preparation of periodic statistics reports and other management information system requirements for the Medical Records Supervisor’s office.
  • As the needs of the department change, so must the ability of the Medical Records Clerk to adjust to the tasks assigned to them by the Medical Record Director.
  • The medical record clerk will audit the discharge procedure checklist on a monthly basis and enter data into a monitoring system.
  • Must be able to use good judgment outside of a predetermined set of guidelines and apply knowledge to various factual circumstances.
  • Take charts and loose sheets from everywhere in the clinic or hospital setting and distribute them.
  • The Medical Record Clerk must also locate and make numbered files, file processed medical records, and make corrections to errors as doctors finish charts.
  • Sort and organize loose pages in accordance with established paper and electronic scanning standards.
  • This person must be self-motivated, organized, and able to multi-task, manage sensitive material, and preserve confidentiality while adhering to the Clinic’s philosophy and objectives.


  • Proficient in the use of Microsoft Office programs (Word, Outlook, PowerPoint, and Excel).
  • Experience working as a medical record technician, medical record clerk, unit secretary, or in a job similar to these where processing electronic medical/health records was a regular duty, for a minimum of one year.
  • Ability to work in a rapidly changing work environment requires excellent interpersonal skills.
  • Understanding the fundamental ideas behind common electronic medical record practices, techniques, and necessities to carry out a broad spectrum of everyday medical records management.
  • The ability to spot and act on discrepancies in information.
